It threw an error because “-” was the last item in the list (the only place it’s valid in a character class without being escaped). Escape it and it should work fine.

I found the line that contains RewriteRule ^([A-Za-z0-9-]+)/?$ categories.php?name=$1 [L] to be particularly useful, however it doesn't include underscores. The syntax is a little different than php's so I couldn't just add _ to the end of the character class without having apache throw me a 500 internal server error. So a simple alternative is to do: ^(([A-Za-z0-9-]|\_)+)/?$
